ChooseHealth is a special program for PHP Care Complete FIDA-IDD Plan members. At Partners Health Plan, we want you to live your best life and to support this, we are offering a Wellness and Rewards Program called ChooseHealth! This program can motivate you to maintain healthier behaviors while helping you combat preventable diseases!

Partners Health Plan members are rewarded, at no cost, through PHP's ChooseHealth program, just by completing two of the following goals:

How can you the benefits of the Choose Health program? The Choose Health program is for qualified PHP Care Complete FIDA-IDD Plan members. Members can contact their care team for more information.
