Partners Health Plan (PHP) provides a person-centered health plan, created to specifically support adults who have Intellectual and Developmental Disabilities.
Who We Are
PHP is the only IDD-specialized health plan in New York, providing comprehensive person-centered services, and is where adults with Intellectual and Developmental Disabilities get the support they need and deserve.
Partners Health Plan (PHP), a not-for-profit managed care organization, is dedicated solely to providing supports and services for persons with Intellectual and other Developmental Disabilities (IDD). The plan is now governed by all five Downstate AHRCs/ARCs and ADAPT Community Network.
PHP was developed by the same providers who have effectively served individuals with IDD in the community for decades and we are committed to employing a person-centered approach to care planning and service delivery that addresses the needs of the whole person. Planning and developing this model has taken several years, and PHP is the culmination of a rich and robust collaboration among a diverse group of providers and other stakeholders dedicated to serving this population.
What We Do
We know that health care can be confusing.
With more than six decades of experience serving persons with IDD, PHP has the expertise to develop and coordinate a personalized Life Plan that ensures participants receive the essential medical, behavioral, dental, habilitation, and other services and supports needed to support each participant’s health, safety, personal preferences, and valued outcomes. We understand the needs of our participants and we work closely with local providers and community groups to meet those needs.
We also know that health care can be confusing. That’s why a highly qualified clinical team leader and care manager will be assigned to help you and your family or caregiver. We want to be sure you and your family and caregivers are off to a good start. This website is designed to help you understand your managed care plan and its many benefits.

PHP’s objective is to provide a seamless array of supports and services with the individual at the center. We are dedicated to keeping each of our participants healthy, happy, and as independent as possible while they pursue their dreams and valued outcomes in the diverse communities they call home. Working in close collaboration with families and other members of the community, PHP will continue the proud ARC/ADAPT tradition of supporting the unique needs and wishes of the individuals in their care, fully integrating their supports and services, advocating on their behalf, and assisting participants throughout their lives. Our goals are lofty, but so are the expectations of the people we are committed to supporting.

Our Vision
Partners Health Plan is New York State’s only fully integrated duals advantage plan dedicated to serving individuals with intellectual and developmental disabilities and their families.
By combining coverage and provider networks for Medicare, Medicaid, and IDD services, PHP is able to provide its members a cohesive, easy-to-navigate plan of care that is tailored to each individual’s needs.
Partners Health Plan is also able to draw from the support and experience of its long-established and trusted sponsor agencies: NYSARC and ADAPT Community Network (formerly United Cerebral Palsy of New York City), which are both leading providers of programs and services for people with developmental disabilities. With this strong backing and singular focus on the IDD community, Partners Health Plan is the premier health/IDD services plan in New York State.

Our History
In recent years, recognizing the sometimes fragmented delivery of services and supports for individuals with IDD, NYSARC, its downstate Chapters, and ADAPT partnered together to form a first of its kind comprehensive managed care plan to cover health and IDD services for individuals with developmental disabilities and their families.
Partners Health Plan, New York State’s first managed care plan exclusively dedicated to serving persons with intellectual and other developmental disabilities, was a result of this partnership. Through its PHP Care Complete FIDA-IDD Plan, Partners Health Plan provides all Medicare- and Medicaid-covered services, including OPWDD services to eligible individuals who elect to participate in New York City and Nassau, Suffolk, Westchester, and Rockland Counties.
