Ride Health - 2025 Non-Emergency Benefit Guidelines

Covered Services

Medical Appointments & Pharmacy

Benefit Limit: Unlimited

Religious Services

Benefit Limit: Unlimited

Grocery Shopping

Benefit Limit: 120 round trips per year, 10 miles each way

Community Activities

  • Volunteering, School/Educational Programs, Supported Employment (SEMP)
  • Community Events: Hosted by PHP, DD agencies, religious groups, local/state/county/community district/board, events open to the public or community residents including local fundraisers, community festivals, Town Hall, Local Council, and/or Civic events open to the public or community residents.

Benefit Limit: 24 round trips per year

Social Services

  • SSI, LDSS, Local Medicaid Office

Benefit Limit: 4 round trips per year

Excluded Services

  • Day Habilitation
  • Alternative Health Care
  • Experimental Medical Procedures/Drugs
  • Community Habilitation
  • Cosmetic Surgery
  • Infertility Services
  • Pre-Vocational
  • Lead Screening/Testing
  • Trips out of PHP’s 9-county service area, unless approved by Care Management
  • Out of State Trips
  • Trips that do not fall into any of the above covered service categories

Other Guidelines

  • Non-Emergency Transportation (NEMT) Benefit
    • Benefit available to Community members only
  • Exceptions for Residential Members
    • Hospital discharge to Skilled Nursing Facility (SNF) or Rehab facility (not back to OPWDD certified residence)
    • Adult Day Health Care (ADHC)
  • Mileage Limits
    • Soft Limit: 25 miles, can be overridden by Care Team
    • Hard Limit: 100 miles, cannot be overridden by Care Team
  • Booking Trips
    • Call Center: 1-844-982-3194 or contact your Care Manager for assistance
    • Book 48 – 72 hours in advance

Summary of Changes from Benefit Year 2024 to Benefit Year 2025

  • SEMP added as a valid Community Activity, included in the 24 round trip benefit limit per year
  • Grocery store trips now have a benefit limit (120 round trips per year, 10 miles each way)
  • Effective January 1, 2025 PHP members will not be able to use the Ride Health portal to book their trips. Members should contact the Ride Health member services line at 1-844-982-3194 or work with their Care Team to schedule their rides.
